Acknowledgments
We would like to thank all of the contributors to this book—their hard 
work and time on top of their very busy schedules has now made the field 
of neuromodulation even better. We specifically wish to give a special 
acknowledgment to Paul Kalanithi, MD, who even during an illness that 
recently took his life was still able to contribute a chapter here.
We also now acknowledge our Elsevier cohorts, Kristi Anderson and 
Natalie Farra, for all of their reminders and work behind the scenes 
during this process. We have worked with them on two books now and 
appreciate their endless patience. Finally we want to thank our families, 
Kelly, Chad, Tyler, Alexis, and Juliane, for all of their support during the 
editing of this book. We are all busy all of the time it seems, but having 
them there and in our lives still matters the most.
