Farmacocinética de un Fármaco La amoxicilina, una penicilina semisintética sensible a la penicilinasa, tiene una estrecha relación con la ampicilina en aspectos químicos y farmacológicos. Propiedades Farmacoterapéutico Amoxicilina La amoxicilina es estable en ácido, diseñada para uso oral, y se absorbe más rápido y de manera más completa del tracto GI que la ampicilina. Las concentraciones máximas de amoxicilina en plasma son de 2 a 2.25 se alcanzan en un plazo de 2 h y son de unos 4 mg/ml cuando se administran 250 mg. Propiedades químicas Amoxicilina Farmacodinamia Amoxicilina Actúa inhibiendo la síntesis de la barrera de peptidoglicano de la pared celular bacteriana al unirse e inactivar a las enzimas implicadas en el proceso. Es un antibiótico betalactámico con actividad antimicrobiana intrínseca insignificante y elevada afinidad por enzimas betalactamasas producidas por algunas bacterias que degradan por hidrólisis a la amoxicilina Gram positivos y Gram negativas productoras y no productoras de betalactamasa, como: Gram (+): Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus spp. (incluyendo Streptococcus pneumoniae y Streptococcus pyogenes) y Corynebacterium spp. Gram (-): Haemophilus influenzae, Haemophilus parainfluenzae, Moraxella catarrhalis, Klebsiella spp., Enterobacter spp., Neisseria gonorrhoeae, Pasteurella mutocida, Escherichia coli, Proteus mirabilis, Salmonella spp. y Shigella spp. Farmacocinetica Amoxicilina Se absorben bien y casi completamente en el tracto gastrointestinal, alcanzando concentraciones plasmáticas pico a las 1-2.5 horas de su administración oral. Proteínas plasmáticas en un 17-20% Se metaboliza parcialmente por hidrólisis al ácido penicílico que se excreta, junto a un 50-75% de fármaco inalterado, por la orina mediante secreción tubular y filtración glomerular, y en muy escasa proporción por las heces. La vida media de eliminación es de 1 hora.
