Para el tratamiento de las FDC, al presentar estas múltiples aferencias de pequeño calibre, se prefiere en cambio el abordaje por vía venosa hasta el seno cavernoso, utilizando espirales de platino de los di- ferentes tipos para rellenarlo y lograr su trombosis con cierre de la fístula. En cambio, para el tratamiento de los AIC se puede afirmar que si bien la oclusión del vaso portador no re- presenta una solución ideal, el atrapamiento carotídeo, previa prueba de tolerancia para aneurismas displási- cos y/o gigantes, continúa siendo un tratamiento de gran fiabilidad. Mientras tanto, y hasta no contar con resultados a largo plazo de los stents de malla cerrada, la embolización selectiva de los AIC utilizando solo espirales, que se incrementa al utilizar la técnica de reconstrucción arterial con stents diseñados para tal efecto, aún es el patrón de oro (gold standard), con altas tasas de oclusión satisfactoria y remisión clínica total o parcial en todos los casos 15,75, probablemente debido a la cesación del fenómeno de latido arterial sobre los pares craneanos afectados.
