Los tumores en etapas II a IV son raros, pero las pacientes con estos cánceres tienen un mal pronóstico, similar al de sus contrapartes epiteliales. Por desgracia no se han observado mejorías en la supervivencia en los SCST ováricos en las últimas décadas (Chan, 2006). De los factores clínicos que afectan el pronóstico, la etapa qui- rúrgica y la enfermedad residual son los más importantes (Lee, 2008; Zanagnolo, 2004). Además, en un estudio de la base de datos Suveillance, Epidemiology and End Results (SEER), Zhang et al. (2007) realizaron un análisis de variables múltiples en 376 mujeres con SCST y concluyeron que la edad menor de 50 años también era un factor predictivo independiente de mayor super- vivencia. ■ Tratamiento durante el embarazo Raras veces se detectan SCST ováricos durante el embarazo (Okada, 2004). En un estudio basado en la población de California que incluyó a más de cuatro millones de pacientes obstétricas, se diagnosticó un tumor de células de la granulosa entre 202 mujeres con una neoplasia maligna ovárica (Leiserowitz, 2006). Los tumo- res de células de la granulosa son los SCST más frecuentes, pero sólo 10% se diagnostica durante el embarazo (Hasiakos, 2006). La presencia de SCST se diagnostica en un tercio de las muje- res embarazadas en forma incidental en la operación cesárea, un tercio presenta dolor o infl amación abdominal y el resto puede presentarse con hemoperitoneo, virilización o hemorragia vaginal (Young, 1984). El tratamiento quirúrgico debe ser el mismo que para una paciente no embarazada. En la mayor parte de los casos el procedi- miento primario es conservador con USO y estadifi cación quirúr- gica, aunque la histerectomía y la BSO están indicadas en algunas circunstancias (Young, 1984). En la institución donde ejercen los autores, la quimioterapia posoperatoria casi siempre se pospone hasta después del parto porque los SCST tienen un patrón de cre- cimiento indolente.
