Placenta 24:797, 2003 las pacientes con quimioterapia citotóxica, histerectomía o ambas, cuando en realidad no existe β-hCG o enfermedad trofoblástica (Cole, 1998; Rotmensch, 2000). Este resultado “falso” se debe a la presencia de anticuerpos heterófi los en el suero que interfi eren con el inmunoensayo para detectar la β-hCG y producen un resultado positivo falso (Soper, 2004). Hay varias formas de aclarar el diagnóstico. Primero, puede realizarse una prueba urinaria de embarazo. En caso de una lec- tura falsa del nivel de β-hCG, los anticuerpos heterófi los no se fi ltran ni excretan por vía renal. Por tanto, la prueba urinaria tiene resultados negativos para β-hCG. Un dato importante es que para excluir la enfermedad trofoblástica de manera concluyente con este método, la concentración índice de la β-hCG sérica debe ser mucho más alta que el umbral de detección de la prueba uri- naria. La segunda es realizar diluciones en serie de la muestra de suero, lo que produce un descenso proporcional en el nivel de la β-hCG si ésta en realidad se encuentra presente; las mediciones de la β-hCG falsas no se modifi can con esta técnica. Además, en caso de sospecha algunos laboratorios especializados pueden bloquear los anticuerpos heterófi los. Por último, un ensayo distinto para detectar la β-hCG con un método alterno podría demostrar con precisión la ausencia de hormona verdadera (Cole, 1998; Olsen, 2001; Rotmensch, 2000). ■ Enfermedad trofoblástica gestacional quiescente Las pacientes con elevaciones leves persistentes (casi siempre alrede- dor de 50 mUI/ml o menos) de la β-hCG verdadera pueden tener un trastorno premaligno latente si no se identifi ca un tumor en la exploración física o en los estudios de imagen (Khanlian, 2003). En tal caso, también debe descartarse con certeza la posibilidad de una lectura falsa de la concentración de β-hCG. Los niveles bajos de esta hormona pueden persistir durante meses o años antes de desaparecer. Por lo general la quimioterapia y la cirugía no tienen efecto alguno. La anticoncepción hormonal puede ayudar a dismi- nuir los títulos a concentraciones indetectables, pero las pacientes deben mantenerse bajo vigilancia estrecha porque es posible que desarrollen GTN metastásica (Khanlian, 2003; Kohorn, 2002; Palmieri, 2007).
