Resultados: Trece hospitales participantes: 6 en ThinPrep, 5 en triple toma y 2 en SurePath. La concordancia global media fue del 69,7%. La concordancia media por tipo de lesión fue del 41,7%. La concordancia en negativos fue del 78,1%, en ASCUS del 41,3%, en LSIL de 58,9% y en HSIL del 64,2%. Los casos discordantes se diagnosticaban con mayor frecuencia como negativos, ASCUS y LSIL. Las discordancias severas (HSIL/ASCH versus negativo), alcanzaron un 3,8% de casos. Conclusiones: Nuestros resultados son similares a los descritos en la literatura con muy esca- sas discordancias severas. El método de intercambio de laminillas utilizado no permite una formación continuada posterior, ya que no incluye la revisión de los casos con discordancias diagnósticas. Results of the first round Abstract Introduction: The Spanish guidelines for cervical cancer screening recommend participating in external quality control programs. The Spanish Society of Cytology has started its own program of quality control for gynecological cytology. Aim: To describe and discuss the results of the first round of The Spanish Society of Cytology quality control for gynecological cytology. Material and method: Organization of participating hospitals on the basis of cytology type. Exchange of diagnostic slides from 50 cases among participants, as follows: 10 negative; 20 ASCUS-ASCH, 10 LSIL and 10 HSIL. Analysis of inter-observer correlation. Results: 13 hospitals took part: 6 with ThinPrep liquid-based cytology, 5 with conventional cytology, and 2 with SurePath liquid-based cytology. The global mean concordance was 69.7%. The mean concordance in the type of lesion was 41.7%. The concordance was 78.1% in negative diagnoses, 41.3%, in ASCUS, 58.9% in LSIL and 64.2% in HSIL The discordant cases were diagnosed more frequently as negative, ASCUS and LSIL. 3.8% of cases showed major discordances (HSIL or ASCH versus negatives). Conclusions: Our results are similar to those reported in the literature, with very little severe discordance. The method of exchanging slides does not allows continuous training, since the review of discordant cases can not be made. Introducción La citología ginecológica aplicada en los últimos años, si bien de forma oportunista y con periodicidad anual, nos ha lle- vado a una disminución de la incidencia de cáncer de cérvix en el mundo occidental, de manera que, en algunos países, España entre ellos, el cáncer de cérvix ha dejado de ser el primero en incidencia, dando paso a otras neoplasias como las de mama, colon y pulmón. El conocimiento acerca de la biología evolutiva del cán- cer de cérvix y la importancia capital de la infección por VPH como condición «sine qua non» para el desarrollo de este cáncer, así como la evidencia de la relativamente baja sen- sibilidad de la citología para el diagnóstico de lesiones CIN2+ (neoplasia intraepitelial cervical de grado II o más), han lle- vado a la recomendación de establecer un nuevo método de cribado de cáncer de cérvix en España1. Por lo que respecta a la citología ginecológica, dichas recomendaciones incluyen la práctica de controles de cali- dad internos en los laboratorios de citología,en especial a la correlación citohistológica y al estudio de los fal- sos negativos2---7. En la literatura actual, la tasa de falsos negativos se encuentra alrededor del 7,8%, de los cuales aproximadamente la tercera parte corresponde a un pro- blema de la toma, otra tercera parte a un error de lectura y una tercera parte a errores de interpretación7. Todo ello pone de manifiesto la importancia de la formación conti- nuada. En este sentido, se ha propuesto que una de las herramientas de formación podría ser la revisión del atlas de Bethesda en los propios laboratorios, pero según algu- nos autores, esta práctica no parece útil para mejorar la interpretación de los casos8. La experiencia recogida en la literatura incluye diversos modelos de controles de calidad que abarcan el estudio de la calidad de la muestra, la calidad tintorial y estudios de la calidad del diagnóstico9---14. Uno de estos modelos está realizado con intercambio de preparaciones15. En ese estudio, entre otros detalles, el error serio o grave quedaba definido como aquellas situaciones en las que se observaba una lesión de alto grado, o cáncer que no había sido recogida en el informe citológico. Se han desarrollado también métodos basados en la digi- talización de preparaciones, pero solo pueden utilizarse con citología en monocapa16,17. En los estudios comparativos, uno de los caballos de batalla es el diagnóstico de atipia en células escamosas de significado incierto (ASCUS) ya que frecuentemente se utiliza como cajón de sastre y no con criterios citológicos estrictos. En esta área existen distintos trabajos que esta- blecen porcentajes de correlación interobservador si bien la mayoría, tienen también en cuenta el resto de diagnósticos citológicos17---22. La Sociedad Española de Citología (SEC) está desarro- llando un programa de control de calidad de la citología ginecológica. En la primera fase hemos procedido a efectuar un estudio de correlación entre observadores (interobserva- dor). Objetivo Nuestro objetivo en este trabajo es, por un lado, dar a conocer el programa de control de calidad de la citología ginecológica que efectúa la SEC y, por otro, mostrar los resultados del estudio de correlación interobservador en la primera ronda efectuada. Material y métodos La SEC ofrecía a sus socios la participación en el presente estudio en la forma siguiente: Cada uno de los participantes debía en primer lugar mostrar su voluntad de participación, inscribiéndose en el programa. Se solicitaban, entre los datos de inscripción, la descripción del tipo de citología utilizada (ThinPrep, con- vencional o SurePath) así como una persona de contacto, responsable del control de calidad. Dicha persona debía escoger un total de 50 casos, incluyendo, 10 negativos para células malignas, 20 ASCUS, atipia en células escamosas sos- pechosa de lesión de alto grado (ASCH), lesión escamosa intraepitelial de bajo grado (10 LSIL) y lesión escamosa intraepitelial de alto grado (10 HSIL). A su vez, la SEC, estableció un «centro de coordinación» del programa. Una vez conocidos los hospitales participantes y el tipo de citología, se establecieron tres ruedas de intercambio de casos, asociadas a los distintos tipos de citología utilizada, procediendo de la siguiente forma (fig. 1): El hospital A enviaba los casos seleccionados al hospital B y los diagnósticos de estos casos al centro de coordinación de la SEC. El hospital B recibía los casos, los diagnosticaba y enviaba sus diagnósticos al centro de coordinación de la SEC a la vez que devolvía los casos al hospital A. A su vez el hospital B hacía lo mismo con el hospital C, el C con el D, y así sucesivamente hasta cerrar el círculo de hospitales participantes de manera que el último hospital procedía con el primero en la forma indicada. Una vez recogidos todos los datos, el centro de coordina- ción de la SEC calculaba la concordancia entre observadores para cada hospital, la media de la concordancia entre observadores para cada tipo de citología y la media de la concordancia entre observadores global, datos que se trans- mitían a los hospitales participantes y que se exponen en este trabajo. Se calculaba también la concordancia entre observadores en casos negativos, y por tipo de lesión (ASCUS, ASCH, LSIL y HSIL). También se analizaban las discordancias estableciendo una estratificación en relación con la importancia clínica, subdividiendo entre discordancias severas mayores, meno- res e intermedias. Las discordancias severas se definieron como aquellos casos informados como HSIL o ASCH y que en la revisión se informaron como negativos. Las discordancias mayo- res, se definieron como aquellos casos informados como LSIL o ASCUS y que en la revisión se informaron como negativos. Las discordancias menores se definieron como aquellos casos diagnosticados como HSIL que en la revisión fueron informados como ASCH y aquellos casos informa- dos como LSIL que en la revisión fueron informados como ASCUS. El resto de discordancias fue recogido bajo el grupo de discordancias intermedias. Las concordancias y discordancias se calcularon respecto al diagnóstico inicial establecido por cada uno de los hospitales participan- tes que había seleccionado los casos, y que los enviaba al segundo hospital. No se estableció ningún «comité de expertos» que corroboraran dichos diagnósticos de manera que solamente se estudió la concordancia o discordancia entre observadores sin estándar de referencia (gold están- dar). Resultados Participaron un total de 13 hospitales, 6 en la ronda de citología líquida ThinPrep, 5 en la ronda de citología con- vencional con triple toma y 2 en la ronda de citología líquida con SurePath. La tabla 2 muestra el total de concordancias y discordan- cias. Para este estudio se han distribuido el total de casos estudiados, por diagnóstico inicial y por tipo de citología. Así por ejemplo, como preparaciones negativas, se revisaron un total de 20 de SurePat, 60 de ThinPrep y 52 de convencional. Se observa pues que si bien todos los participantes aportaron el mismo total de casos, no todos aportaron exactamente el mismo número de casos para cada diagnóstico. En la tabla se muestran el total de casos concordantes y discor- dantes organizados por diagnósticos iniciales y por tipo de citología. La concordancia media en diagnósticos negativos fue del 78,1%, y fue superior en el grupo con citología ThinPrep. La media de discordancias en casos negativos fue de un 21,9%. La mayoría de los casos discordan- tes se agruparon en el grupo de ASCUS 14,6% de casos (tabla 2). La concordancia en ASCUS fue del (41,3%), Las discordan- cias alcanzaron un 58,7% de casos. El diagnóstico inicial de ASCUS fue interpretado como negativo en un 23,9% de los casos y como LSIL en un 20,5% de casos (tabla 2). En LSIL, la media de concordancia fue de 58,9%. Las discordancias alcanzaron un 41,1% de casos. El diagnóstico inicial de LSIL fue interpretado como ASCUS en un 21,1% de casos (tabla 2). La media de concordancia en ASCH fue del 19,8%. Las discordancias alcanzaron un 80,2% de casos. El diagnóstico inicial de ASCH, como ASCUS en un 44,3% de casos y como HSIL en un 29,4% de casos fue el responsable de esta baja concordancia interobservador (tabla 2). La media de concordancia en el diagnóstico de HSIL fue del 64,2%, por tanto las discordancias alcanzaron un 35,8% de casos. EL diagnóstico inicial de HSIL fue interpretado como negativo en un 5,6% de casos, como ASCUS en un 7,7% de casos, como ASCH en un 11,9% de casos y como LSIL en un 10,6% de casos (tabla 2). En cuanto a la distribución de las discordancias según las categorías previamente definidas, los resultados fueron los siguientes: la media de discordancias severas (HSIL o ASCH versus negativo) fue de un 3,8%. Discusión El control de calidad de la citología ginecológica es una de las recomendaciones de las guías de cribado de cáncer de cérvix que efectúan las distintas sociedades, entre ellas la SEAP, la SEGO, la AEPCC y la SEC1. Además, se recomienda la implementación de contro- les de calidad internos dentro de los servicios de anatomía patológica y de los laboratorios de citología, dirigidos princi- palmente a reducir la tasa de falsos negativos y a establecer una correlación citohistológica. Se han publicado diversos sistemas y estrategias en la literatura2---5,7,10 pero el con- trol de calidad interno no es objeto de este trabajo. Las guías recomiendan también la participación en controles de calidad externos a los propios servicios, dirigidos fundamen- talmente a la reproducibilidad de la orientación diagnóstica de la citología1. Existen pocos trabajos publicados en rela- ción con el control de calidad externo y hacen referencia a distintas estrategias adoptadas por los organismos oficiales de salud14,15. La SEC ha procedido a la implementación de un programa de control de calidad de la citología ginecológica, dirigido por la propia Sociedad y con participación voluntaria de distintas instituciones hospitalarias. En una primera fase se ha procedido al estudio de la reproducibilidad diagnóstica entre cada una de las instituciones participantes sin acordar previamente cuál era el diagnóstico «verdadero» en cada caso. Debido a que los distintos procedimientos técnicos existentes para el procesamiento de la muestra citológica introducen diferentes artefactos en las extensiones, y que ello puede teóricamente inducir a errores de interpretación, se decidió organizar la primera ronda del programa en fun- ción del tipo de método citológico utilizado. De esta forma se organizó el intercambio de casos sin mezclar los tipos de preparación citológica. Los resultados expresados en este trabajo hacen refe- rencia a tres tipos de citología y a la media global entre ellos. La SEC ha orientado la primera fase de control de cali- dad comparando los equipos diagnósticos, ya que es de esta forma como se trabaja usualmente en los distintos labora- torios de citología. Otros estudios publicados respecto a control de cali- dad externo, evalúan la presencia de células endocervicales y metaplásicas6, la calidad de la muestra, la calidad tintorial14, así como el índice de falsos negativos en carcinomas6. Rara vez se han realizado controles de cali- dad en relación con la correlación diagnóstica, comose expresa en el presente trabajo. En algunos estudios la corre- lación diagnóstica llega al 96%13. Otros trabajos observan un incremento de la correlación diagnóstica a lo largo de los años en relación con la participación en controles de cali- dad externos12. Este dato aboga en favor de la participación sistemática en programas de control de calidad externos, ya que la participación en los mismos, puede incrementar la calidad diagnóstica, y servir como formación continuada a los participantes. Solamente hemos encontrado en la literatura una expe- riencia de intercambio de preparaciones similar al descrito en este trabajo15. En él definían también discordancia seria o grave cuando se observaba una lesión de alto grado, cán- cer o neoplasia glandular que no había sido informado. Es lo que ha sido llamado una discordancia severa en el presente trabajo. El estudio referido no definía los demás tipos de discordancias. Es importante también evaluar los aspectos de forma- ción continuada que tiene el control de calidad. En general se establece una relación entre el esfuerzo de participa- ción y el beneficio obtenido. En el sistema de intercambio de preparaciones no se puede revisar la preparación con diagnóstico discordante dado que la misma se devuelve al hospital de origen. De esta forma, no es posible establecer discusión ni un proceso de aprendizaje a partir de los casos. En consecuencia, este sistema tiene poco valor educativo. Algo similar sucede con experiencias anteriores15. Nuestros porcentajes en concordancias son similares a los ya expresados en la literatura8,20. En el caso de diagnósticos negativos, nuestra concordancia es del 78,1% siendo en la literatura del 77,6 y del 73,1%18,20. La concordancia en ASCUS (41,3%), es similar a la obser- vada en otros estudios (36,4 a 42,9%). Ello probablemente radica en el hecho de que los criterios que definen el diag- nóstico de ASCUS, si bien son precisos23, no se aplican de forma estricta y esta categoría diagnóstica frecuentemente se utiliza como cajón de sastre. Existen algunos trabajos que aportan concordancias interobservador en ASCUS de aproxi- madamente un 95%,19 y otros de hasta un 97,2%2 pero este dato no se reproduce en nuestra casuística. La mayoría de los casos discordantes lo eran en relación con los diagnós- ticos de negativos y LSIL de acuerdo también con los datos recogidos en la literatura18,20. La concordancia en LSIL es, en nuestro estudio, del 58,9%, oscilando en la literatura18 entre un 52,9 y un 68,1%. La mayoría de casos discordantes lo eran en relación con el diagnóstico de ASCUS como se ha citado en la literatura18, si bien algunos autores20 muestran mayores discordancias con el diagnóstico de HSIL. Llama la atención la discrepancia diagnóstica con ASCUS. La concordancia en HSIL es la más variable en la mayor parte de los estudios, que la sitúan entre un 47,118 y un 83,4%20, y alcanza un 64,2% en nuestro estudio. Los casos discordantes se agrupan en el diagnóstico de LSIL tal y como se muestra también en otros trabajos18,20. En la presente serie las discordancias de los HSIL, también se agrupan en el diagnóstico de ASCH. Después de este análisis cabría decir que la formación continuada debería hacer énfasis en el diagnóstico citológico de ASCUS para mejorar su reproducibilidad. En cuanto a las discordancias con HSIL, este es un hecho ya descrito en la literatura. En un estudio de 100 casos con CIN 3 diagnosticados con biopsia, se valoró el diagnóstico citológico previo, hallando falsos negativos en 26 casos de HSIL. De estos, se observó retrospectivamente que en 16 casos se podría haber realizado el diagnóstico de HSIl21. Se ha estudiado también la frecuencia de concordancias interobservador en los distintos tipos de muestras citológi- cas, resultando bastante similares (tabla 2). Creemos que la evaluación precisa de esta concordancia no es posible dada la diferencia de experiencia entre los distintos observado- res y el escaso número de participantes en algunas de las categorías. La tabla 3 muestra las discordancias, agrupadas en severas, mayores, intermedias o menores. La crítica que se puede hacer a este modo de agrupar las discordancias es clasificar como discordancia mayor a las discordancias entre ASCUS y negativo o LSIL y negativo. Aunque la definición de una discordancia ASCUS- negativo definida como «mayor» pudiera parecer dudosa, cabe recordar que detrás del diag- nóstico de ASCUS existe un porcentaje variable (entre el 5 y el 10%) de HSIL según las publicaciones en este campo. Algo parecido sucede con el diagnóstico de LSIL. Si aplicamos las recomendaciones actuales en caso de usar la citología como cribado inicial ante un diagnóstico de ASCUS está recomen- dado realizar un test de VPH mientras que el manejo clínico recomendado en una mujer con un diagnóstico citológico negativo es el control citológico a los tres años1. La media de discordancias severas es del 3,7%. La media de discordancias mayores es de un 11,9%. Aunque las discor- dancias intermedias pueden conllevar distintos manejos de las pacientes, este grupo de discordancias no implica que se deja de controlar a las pacientes durante un periodo de tres años tal y como sucedería en el caso de las discordancias mayores y severas. Las discordancias menores deben infor- marse, fundamentalmente con un objetivo académico, pero probablemente revisten poca importancia clínica. Las principales debilidades de este estudio radican en el escaso número de participantes por cada tipo citológico, impidiendo un análisis profundo de su impacto y en la ausen- cia de un estándar de referencia potente como la biopsia o el diagnóstico citológico consensuado. Conclusiones Los controles de calidad externos, si bien representan un esfuerzo adicional para el personal del laboratorio de cito- logía, pueden aportar beneficios. Nuestros datos de este estudio son similares a los recogidos en la literatura, desta- cando nuestro bajo porcentaje de discordancias «severas». Sin embargo el método descrito carece de valor en el aspecto de la formación continuada. Para futuras rondas del control de calidad externo de la SEC, deberían establecerse cambios metodológicos que permitan valorar y detectar otros aspectos relacionados con la calidad, y que puedan ser causas de errores diagnósticos.
