Con respecto a la obesidad, los resultados son con-
dictorios. Una revisión sistemática3 admite que los

En cuanto al mayor riesgo de RD en pacientes insulin-
dos, no es atribuible a factores como años de evolución
mal control de la DM, una vez que estos han sido
stados en un análisis multivariante mediante regresión
ística. Esta asociación se ha observado en otros estu-
s manteniendo OR elevadas2,4. En un estudio de cohortes
tuvieron que el grupo de pacientes mal controlados,
 que se insulinizaron tenían más RD (11,3%) durante el

plicación de este hallazgo. Lo que en ningún caso se ha

En cuanto al papel del tabaco dentro de las variables
estudiadas se recogió el tabaquismo, siendo eliminado en el
análisis univariante por no asociarse a la RD (p = 0,882).
Parece, por otros estudios4 y nuestros resultados, que el
papel del tabaco a nivel microvascular no es, al menos, tan
relevante como a nivel macrovascular.

curso clínico como en la reducción de complicaciones y en 
evolución de los pacientes que han sufrido o sufrirán COVID
19 en el futuro. Así, tanto la intervención nutricional como 
terapia farmacológica deben ser consideradas parte integr
del enfoque para los pacientes afectos por SARS-CoV-2 en 
atención médica en general.
La mayoría de los métodos de cribado clínicos (MUS
NRS-2002, MNA, VSG, OST) suele incluir datos subjetivo
y objetivos (peso, talla, cambios en el peso, cambios e
la ingesta y comorbilidades). En los últimos años se ha
desarrollado y validado en España 2 métodos automatizado
conocidos como CONUT e INFORNUT5.

Desde el inicio de la alerta por SARS-CoV-2 se han notificado
en España 213.435 casos de COVID-19 diagnosticados por
PCR1. Se estima una prevalencia mundial muy alta, siendo
imperativa una evaluación adecuada, así como el desarrollo
de una apropiada estrategia de manejo2.
Estudios realizados en otros países han mostrado que el
27,5% de los pacientes hospitalizados presentaban riesgo de
desnutrición y el 52,7% algún grado de desnutrición3.
La COVID-19 se asocia con resultados negativos en
pacientes mayores, con presencia de comorbilidades,
sujetos inmunocomprometidos y personas desnutridas en
general4.
Teniendo en cuenta que el estado nutricional es un factor
relevante que influye en la evolución de los pacientes con
COVID-194, su preservación y tratamiento en los casos de
desnutrición desempeña un papel importante, tanto en el
El CONUT está basado en datos analíticos que incluye
albúmina, colesterol total y linfocitos totales e identifica
de forma sencilla y rápida pacientes en riesgo nutricional,
especialmente de interés en los pacientes con COVID-19.
En la figura 1 detallamos la evolución del riesgo de
desnutrición de un paciente con diagnóstico de COVID-19,
analizando mediante el método CONUT los días 1, 3, 5 y
12 de su ingreso, pudiendo ver la buena evolución de la
enfermedad con una disminución progresiva del riesgo de
desnutrición, de 9 a 1, siendo dado de alta sin riesgo de
malnutrición.
Considerando el número total de afectados registrados,
un alto porcentaje de pacientes con COVID-19 han sido
valorados y seguidos en la fase inicial por atención pri-
maria, a lo que se sumará el seguimiento de los pacientes
hospitalizados dados de alta. Dada la sobrecarga de activi-
dad por la COVID-19 en atención primaria, es tiempo de
adecuar la organización y valorar la modificación de los
protocolos de las enfermedades crónicas para mejorarla
