Cuando queremos establecer una comparación entre dos sustantivos, utilizamos los adjetivos en su grado comparativo (comparative adjectives). Por ejemplo: Smaller incisions reduce the risk of bleeding and infection. (Incisiones más pequeñas reducen el riesgo de sangrado e infección). The advantages of laparoscopic cholecystectomy include less scarring and better wound healing. (Las ventajas de la colecistectomía incluyen menos cicatrización y mejor curación de la herida.) Podemos decir que hay dos clases de adjetivos comparativos: los irregulares y los regulares. Adjetivos irregulares Como podemos observar en el cuadro, el cambio de estos adjetivos no obedece a ningún patrón o regla, y de ahí su clasificación- irregulares. Adjetivo grado comparativo good (bueno) better (mejor) bad (malo) worse (peor) far (lejos) further /farther (más lejos) Adjetivos regulares Los adjetivos comparativos regulares se pueden dividir en dos categorías: los adjetivos cortos y los adjetivos largos. Adjetivos cortos Hay varias reglas para formar los adjetivos comparativos. Regla 1: Si un adjetivo tiene una sílaba, se agrega el sufijo -er al final. (short -> shorter) Regla 2: Si un adjetivo termina en la secuencia [consonante, vocal, consonante], se duplica la última consonante antes de aplicar la regla 1. (wet -> wetter) Regla 3: Si un adjetivo termina en -e, se agrega -r al final. (large -> larger) Regla 4: Si un adjetivo tiene dos sílabas y termina en -y, se cambia la y a i y se agrega el sufijo -er al final. Algunos adjetivos de una o dos sílabas también se incluyen en esta categoría. Son los terminados en ING (como exciting o frightening) o los terminados en ED (como burned o curved). Los pronombres objetivos reemplazan a un sustantivo después de un verbo o una preposición con el fin de evitar la repitición del mismo. Hay un prononbre objetivo para cada pronombre personal (o persona gramatical): La diferencia con los Pronombres Sujeto es que éstos normalmente están al principio de las oraciones y son los que realizan la acción. En cambio, los pronombres objeto normalmente van después de un verbo o una preposición. Ejemplos después de verbos: The liver converts some of its stored glycogen into glucose and releases it into the bloodstream. A mosquito carrying the virus passes it to a human by biting them. Ejemplos después de preposiciones: A bruise is caused when blood capillaries in (or just under) the skin are broken and blood leaks from them. ¿Qué significan? Son adjetivos que expresan el grado máximo de una cualidad de un sujeto, distinguiéndolo entre otros de la misma categoría, como “la tasa más alta” (the highest rate) y “los vasos sanguíneos más pequeños” (the smallest blood vessels). ¿Cuáles son las reglas? Hay 6 reglas para utilizar estos adjetivos de forma adecuada: 1. Si el adjetivo termina en –y (no importa si tiene una o más silabas), debemos cambiar la –y por –i y agregar –est: Ejemplo “easy”…. the easiest : el/la más fácil 2. Si el adjetivo es de una sílaba, es necesario agregar –est al final del adjetivo, así: Ejemplo “short” ….the shortest : el/la más bajo/a. 3. Si el adjetivo de una sílaba termina en “consonante”, “vocal”, “consonante” como “big” (grande), debemos duplicar la última consonante y luego agregar la terminación -est: Ejemplo “big” … the biggest : el / lamás grande 4. Si el adjetivo tiene dos o más sílabas, es necesario utilizar la frase “the most” antes del adjetivo: Ejemplo “dangerous”… the most dangerous: el/la más peligroso 5. Si el adjetivo tiene una sola sílaba y termina en –e debemos agregar –st al final del adjetivo: Ejemplo: “Large”….the largest: el/la más grande 9 / 47 6. Existen adjetivos “irregulares”. Los más comunes son: Para tener en cuenta: Observaremos la palabra “the” delante del adjetivo en grado superlativo. Recordemos que el artículo “the” puede significar “la” “el” “las” “los”: Resumiendo: Los adjetivos comparativos establecen una comparación entre 2 sujetos; mientras que los superlativos distinguen a uno por encima de otros. CLASES DE ADVERBIOS - De modo (adverbs of manner) Expresan cómo se realiza una acción. La mayoría suelen terminar en -ly (equivale a la terminación adverbial castellana 'mente' y por lo tanto suelen formarse a partir del correspondiente adjetivo al que se agrega dicha terminación). Ejemplo: careful - carefully (cuidadosamente), easy - easily (fácilmente), etc. aunque existen excepciones: well (bien), aloud (en voz alta), etc. - De lugar (adverbs of place) Indican el lugar donde se realiza una acción. around (alrededor), far (lejos), near (cerca), there (allí), here (aquí), etc. - De tiempo (adverbs of time) Indican cuándo tiene lugar una acción. again (otra vez), at first (al principio), before (antes), soon (pronto), late (tarde), etc. - De frecuencia (adverbs of frequency) Indican con qué periodicidad se realiza una acción. always (siempre), frequently (frecuentemente), never (nunca), sometimes (algunas veces), etc. - De cantidad (adverbs of quantity) Expresan la cuantía de algo. little (poco), much (mucho), too (demasiado), etc. - Ordinales (ordinal adverbs) firstly (en primer lugar), secondly (en segundo lugar), etc. - De grado o intensidad (adverbs of degree) 13 / 47 14 Indican la intensidad con que se realiza la acción. completely (completamente), extremely (extremadamente), greatly (grandemente), etc. - De probabilidad (adverbs of probability) maybe (quizá), perhaps (quizá, tal vez), possibly (posiblemente), probably (probablemente), etc. - De afirmación (adverbs of affirmation) certainly (ciertamente), naturally (naturalmente), of course (por supuesto), surely (seguramente), etc. - De negación (adverbs of negation) no, not (no), never (nunca), not at all (en absoluto) 14 / 47 15 Los conectores Un conector es una palabra (Ejemplo, and: y) o conjunto de palabras (Ejemplo, As a result: Como resultado) que su usa para unir frases u oraciones. La función de estas palabras es organizar el texto de modo coherente (que tiene un significado comprensible, sentido) y cohesivo (que sus ideas tienen relación entre sí). Este tiempo verbal se forma con el verbo modal WILL seguido de un verbo en su forma infinitiva (el verbo principal). Por ejemplo: Different body regions, e.g. hands, feet, head or internal organs will be(estarán) at different temperatures, but the core temperature will vary (variará) by only one or two degrees. Como vemos en la traducción entre paréntesis, el equivalente del futuro simple en el idioma español es el futuro imperfecto (yo amaré, yo temeré, yo partiré). ¡ATENCIÓN! El verbo WILL no tiene una traducción, sino que permite conjugar al verbo que lo acompaña en FUTURO. En el siguiente cuadro vemos la estructura del futuro simple en sus tres formas con la conjugación del verbo “play” (jugar, desempeñar, etc.): En la afirmación también se puede usar la contracción de WILL,‘ll, que se agrega al pronombre; y en la negación, la contracción de WILL NOT, WON’T; por ejemplo: I’ll call an ambulance. She won’t need antibiotics. Pero recordemos que el uso de las contracciones es informal. El futuro simple se utiliza para expresar distintos tipos de acciones futuras: predicciones basadas en nuestra opinión, certezas, promesas, amenazas, decisiones repentinas o instantáneas, la determinación o voluntad del hablante de realizar una acción en el futuro o inclusive ofrecer nuestra ayuda. 18 / 47 19 Condicional tipo 1 Recordemos algunas ideas sobre las oraciones condicionales: La oración condicional expresa dos acciones, donde una acción es consecuencia o resultado de otra. Podemos cambiar el orden de la condición y su consecuencia sin cambiar el significado de la oración. En general, las oraciones condicionales llevan la palabra “if” (si) como conector entre las 2 acciones. El condicional tipo 1 se refiere a una condición/situación posible y su resultado probable. En estas oraciones, la cláusula "if"/condicional se expresa en el tiempo presente simple y la cláusula/proposición principal, en el tiempo futuro simple. Proposición "if"/condicional Proposición principal If + simple present simple future If the diet is deficient in proteins, vitamins and minerals, deficiency diseases, such as anaemia and rickets, will appear. Nota: El mismo ejemplo se puede expresar de la siguiente manera sin cambiar su significado: Deficiency diseases, such as anaemia and rickets, will appear if the diet is deficient in proteins, vitamins and minerals. 19 / 47 20 Se pueden usar algunos verbos modales en vez de “will” para cambiar la probabilidad o expresar una opinión, sugerencia, etc. Ejemplos: If it doesn’t rain, we may go to the beach.(Significado: Con el uso de “may”, el significado de esta frase cambia. Ahora, el hablante reconoce que puede ir a la playa, pero no está tan seguro de si irá.) If it doesn’t rain, we should go to the beach.(Significado: En este caso, el uso de “should” expresa la opinión del hablante.) If it doesn’t rain, we can go to the beach.(Significado: “Can” significa que es posible ir a la playa, pero no indica la probabilidad.) Traducir el texto. 21 / 47 https://www.liveworksheets.com/ex1518675em 22 Verbos modales Los verbos modales son un grupo de verbos que se caracterizan por expresar distintas funciones como habilidad, posibilidad, permiso, obligación, etc. Cada verbo modal tiene su función/es y su significado. Comenzamos con los verbos modales que expresan posibilidad. VERBOS MODALES DE POSIBILIDAD 1. Can (puedo, podés, puede,…) El verbo modal «can» se utiliza para expresar habilidad, posibilidad, y para hacer peticiones o pedir permiso, e incluso puede ser utilizado para hacer una oferta o una sugerencia. Además, en su forma negativa, «can’t »/ “cannot”, puede usarse para expresar imposibilidad o incapacidad. Usualmente es equivalente al verbo «poder» en español. Ejemplos: The bladder can expand to hold about 400 cm3 urine. The urine cannot escape from the bladder because a band of circular muscle- a sphincter- is contracted, so shutting off the exit. Plants can make their own vitamins. The new strain can spread from the intermediate host to humans. 2. Could (podría, podrías, …/podía, podías,…) Este verbo tiene muchos usos en el inglés. Puede ser utilizado para hablar sobre habilidad y permisos en el pasado; puede usarse en preguntas para hacer peticiones, sugerencias u ofertas, pero de una forma más educada o formal. También puede usarse para hablar sobre posibilidad, pero cuando se refiere a una posibilidad menos certera que la expresada con el verbo «can». Su forma negativa es «couldn’t »/ “could not” Ejemplos: In one pandemic of bubonic plague around 1347 over half the population of Europe died. The medicine of the time could not explain it. But it was obvious that a person could get the disease from contact with the victims and their clothing. If the new strain evolves to spread from person to person, a flu pandemic could arise. 22 / 47 23 3. May (ídem could: podría, podrías, etc; puede que) El uso de «may» es pedir permiso en el presente, hacer sugerencias, y hablar de posibilidades. Sin embargo, en el caso de permisos y sugerencias, este verbo es más formal que «can». Ejemplos: Results may vary. Variations in the pH of the reagents may cause their deterioration. 4. Might (ídem may) El verbo modal «might» se utiliza para expresar posibilidad. Puede también ser utilizado como el verbo «may»,aunque habla de eventos que tienen menor posibilidad de suceder. Su forma negativa, «might not » se utiliza para hablar sobre posibilidades pero en forma negativa. Ejemplo: If you need to get your LDL-C-the bad cholesterol- lower Crestor might help. 5. Will El uso principal del verbo modal «will» en inglés es formar el tiempo futuro. Sin embargo, también cuenta con otros importantes usos tales como hablar de deseos, peticiones, demandas, hacer predicciones y suposiciones (probabilidad), hacer promesas y hablar de decisiones tomadas espontáneamente. En el caso de las peticiones y demandas, el uso de «will» no es educado, sino que más bien puede resultar muy casual. Su forma negativa es «won’t »/ “will not”. 23 / 47 24 Ejemplos: One in every three smokers will die as a result of their smoking habit. Those who do not die at an early age will probably be seriously disabled. Vegetarians and semivegeterians who include dairy products in their diets will obtain sufficient protein to meet their needs. Digestion is mainly a chemical process and consists of breaking down large molecules to small molecules. The large molecules are usually not soluble in water, while the smaller ones are. The small molecules can pass through the epithelium of the alimentary canal through the walls of the blood vessels and into the blood. Some food can be absorbed without digestion. The glucose in fruit juice, for example, could pass through the walls of the alimentary canal and enter the blood vessels. Most food, however, is solid and cannot get into blood vessels. Digestion is the process by which solid food is dissolved to make a solution. The chemicals which dissolve the food are enzymes. A protein might take fifty years to dissolve if it is just placed in water but it is completely digested by enzymes in a few hours. The medical term for jaundice in babies is neonatal jaundice. The needle was maneuvered into the joint space using fluorocopy and the dispersal of the injected contrast is monitored with fluoroscopy and/or sequential images. Figure 4.1A – Fluoroscopy, Shoulder Arthrogram Computed Tomography There are a wide variety of indications for contrast in CT. CT has excellent contrast resolution and this feature can be used to detect subtle differences in tissue enhancement and help to detect liver metastases, central tumor necrosis, and other pathologies that rely upon the differential concentration of injected contrast agent. On a daily basis CT uses the largest volume of water soluble contrast agent in the entire Diagnostic Radiology department. Examples of the use of contrast for CT imaging include, arterial phase imaging, CT Angiography (CTA) such as pulmonary arterial imaging for a Pulmonary Embolus (PE) study, 46 / 47 and lung cancer staging to differentiate between lymph nodes, veins, and arteries in the pulmonary hilum. Oral, water soluble,contrast can also be used in the intestinal tract to facilitate visualizing the stomach, small bowel, or large bowel. For CT, bowel contrast must be water soluble as barium creates very prominent artifacts due to the atomic number of barium absorbing a large amount of the administered x- rays. Gas, in the formed of injected carbon dioxide, is used for the performance of CT Colonography. Risk Factors Associated with Contrast Agents Any drug has risk associated with it, as do contrast agents used in medical imaging. A potential acute situation related to water soluble contrast is the risk of an ‘allergic-like’ reaction (anaphylactoid). This reaction is anaphylactoid as it does not meet the defined criteria for a true allergic reaction (i.e. symptom free exposure to the antigen, development of an immune system mediated response to the antigen, anaphylactic reaction to the next exposure to the antigen). Anaphylactoid reactions to water soluble contrast agents can occur with the first exposure to contrast, skip contrast exposures, and can be highly unpredictable ranging from mild physical responses to cardiovascular collapse and death. Although severe anaphylactoid responses to water soluble contrast are very uncommon, a number of risk factors exist that increase a patient’s risk of developing an allergic-like reaction. The most significant risk factor for an anaphylactoid reaction, or any other type of reaction to water soluble contrast agent, is to have had a previous reaction to contrast. Furthermore, having a known allergy to anything else also increases a patient’s risk, but not nearly to the same extent as reacting to previous contrast. Therefore, contrast can likely still be administered if the patient reports an unrelated allergie. For patients that are considered high-risk for an ‘allergic type’ adverse reaction to contrast, one may consider the use of pre-medication to lessen the chance of an adverse effect. The most common pre- medications used are oral corticosteroid supplemented with an oral, non-selective antihistamine. It is still possible that a patient can have an allergic-like or non-allergic type reaction, regardless of whether they have been administered pre-medication, and this is known as a “breakthrough reaction”. There are other relative contraindications to contrast agents that should be taken into consideration including: Severe anxiety disorders; Patients on Beta-Blockers; Sickle-Cell Trait or Disease; and Hyperthyroidism (when using an iodinated contrast medium). There are non-allergic adverse events that may affect the urinary system including, Contrast-Induced Nephrotoxicity (CIN), as well as Nephrogenic Systemic Fibrosis (NSF). 47 / 47
