Doce participantes masculinos con acorta- miento de la BIT y 12 participantes masculinos sin acortamiento de la BIT realizaron una prueba de alteración repentina de los miembros inferiores en dirección anterior, posterior, ipsolateral y contralateral. Se midió la latencia de activación electromiográfica del vasto medial (VM), vasto lateral (VL), tensor de la fascia lata (TFL) y glúteo mayor (GM) durante esta prueba. Se utilizó la prueba t de Student para comparar la latencia muscular entre los grupos. Se consideró un nivel alfa de 0,05. Resultados: Durante una desestabilización anterior se observó un aumento significativo de la latencia del VM (p = 0,0001), VL (p = 0,0163), GM (p = 0,0036) y TFL (p < 0,0001) en los partici- pantes con acortamiento de la BIT. En una desestabilización posterior, se observó un aumento significativo de las latencias de activación de GM (p = 0,0001) y TFL (p <0,0001) en individuos con acortamiento de la BIT. Durante una desestabilización contralateral a la extremidad eva- luada, se observó un aumento de la latencia solo en el TFL (p <0,0001) en participantes con acortamiento de la BIT. Latency delay of hip and knee muscles in individuals with iliotibial band tightness Abstract Objective: To determinate the onset latency of the hip and knee muscles in participants with iliotibial band (ITB) tightness. Material and methods: A case-control study. Twelve male participants whit ITB tightness and 12 male participants without ITB tightness performed a sudden lower limbs perturbation in ante- rior, posterior, ipsolateral and contralateral direction. The electromyographic onset latencies of the medial vastus (VM), vastus lateralis (VL), tensor fascia lata (TFL) and gluteus maximus (GM) were measured during this test. The statistical analysis was performed with t-student test. Significance level was established a priori at P<.05. Results: A significant increase of the VM (P=.0001), VL (P=.0163), GM (P=.0036) and TFL (P<.0001) latency was observed in participants with ITB tightness during a sudden anterior perturbation. In a posterior perturbation, a significant increase in the GM (P=.0001) and TFL (P<.0001) latency was observed in participants with ITB tightness. A significant increase of the TFL (P<.0001) latency was observed in participants with ITB tightness during a contralateral limb evaluated perturbation. All rights reserved. l f t ( e l h c i y ( m d a r c l s a i e e d a m i a Introducción La banda iliotibial (BIT) se origina como un engrosamiento de tejido conectivo de la fascia lata del muslo proveniente de la fascia de los músculos glúteo mayor (GM) y tensor de la fascia lata (TFL)1,2. Aproximadamente 3/4 partes del ten- dón del GM se unen con la BIT2. Distalmente, se separa en 2 bandas: el tracto iliotibial, que es la continuación de la BIT, que se inserta en el tubérculo de Gerdy de la tibia, y la banda iliopatelar, que forma parte del retináculo lateral de la rodilla y se inserta en el borde lateral de la patela1. La BIT tiene otras inserciones distales, que incluyen el tendón pate- lar y los músculos bíceps femoral y vasto lateral (VL)1,3,4. Funcionalmente, la BIT se comporta como una estructura estabilizadora lateral de la cadera durante los movimientos de aducción y rotación interna5. Además, resiste los movi- mientos de traslación anterior y rotación interna de tibia5. Por su relación anatómica con la patela, se le ha atribuido una función indirecta en la estabilización lateral, que actúa como un sistema de retención pasiva del deslizamiento pate- lar medial6. El acortamiento de la BIT ha sido considerado como causa primaria o secundaria de diversas disfunciones de miembro inferior7. Se presume que el acortamiento de la BIT sería una de las causas principales del sín- drome de fricción de la banda iliotibial (SFBIT), ya que una banda más estrecha conduciría a una mayor compresión de los tejidos subyacentes durante activida- des motoras repetitivas que incluyen la flexo-extensión de rodilla8. Además, el acortamiento de la BIT ha sido consi- derado como uno de los principales factores que contribuyen al mal alineamiento patelar debido a las relaciones anató- micas existentes entre la BIT y la patela9. La tensión de la BIT aumentaría el deslizamiento lateral de la patela y c t l e a carga relativa sobre las superficies de contacto entre la aceta patelar lateral y el cóndilo femoral lateral, y con- ribuiría al desarrollo del síndrome de dolor patelofemoral SDPF)6,9---11. Se han reportado alteraciones en la latencia y amplitud lectromiográfica (EMG) de los músculos de cadera y rodi- la en el SFBIT y el SDFP12---16. En individuos con SFBIT se a encontrado debilidad de los músculos abductores de la adera12. Por su parte, los sujetos con SDFP han exhibido un ncremento en la amplitud de activación del músculo VL13 un retraso en la latencia de los músculos vasto medial VM)14 y glúteo medio15,16. Estas alteraciones del control otor serían causadas por el daño y el dolor de tejidos blan- os lesionados17, los cuales podrían estar asociados con el cortamiento de la BIT. Sin embargo, se desconocen las alte- aciones en la latencia de activación de los músculos de adera y rodilla que podrían influir en el acortamiento de a BIT. El método más común para evaluar el control motor con- iste en medir la latencia de activación muscular frente alteraciones impredecibles18. Este método proporciona nformación sobre el rendimiento neuromuscular y permite videnciar si los acortamientos de tejidos blandos influyen n la latencia de activación muscular19. A través de EMG e superficie (EMGs) se puede cuantificar la latencia de ctivación muscular, la cual ha sido definida como la pri- era respuesta motora detectada posterior a una alteración mpredecible20. La tasa e muestreo empleada para almacenar la señal en el com- utador fue de 4.000 Hz, utilizando un convertidor análogo igital con una resolución de 16 bit. Se realizó una prueba de alteración repentina de los iembros inferiores para medir la latencia de activación e los músculos VM, VL, TFL y GM. La localización de os electrodos fue de forma longitudinal a las fibras de os músculos según surface electromyography for the non- nvasiveassessment of muscles (SENIAM)22. En los sujetos sin cortamiento se evaluó la extremidad dominante, mientras ue en los sujetos con acortamiento se evaluó la extre- idad más afectada en caso de tener la prueba de Ober ositiva bilateralmente. Una vez posicionados los electro- os se procedió a realizar la prueba de alteración repentina n dirección anterior, posterior, ipsolateral a la extremidad valuada y contralateral a la extremidad evaluada. Se soli- itó a los participantes que estuvieran de pie sobre un carro óvil con ruedas de una superficie de 50 × 50 cm (fig. 1). obre el carro se ubicó un acelerómetro triaxial (Delsys Inc., oston, MA, EE. UU.) para conocer el instante exacto de la lteración. El carro se encontraba unido a una cuerda de lambre de acero galvanizada de 1/8 pulgadas. La cuerda Latencia muscular en individuos con acortamiento de la banda il estaba conectada a un sistema de poleas donde de forma imprevista se agregaba una carga equivalente al 10% del peso corporal del participante evaluado para generar la desestabilización. Este sistema se encontraba detrás de una cortina de color negro que impedía que el participante eva- luado pudiera detectar el instante de la desestabilización. Los participantes fueron instruidos para mantenerse de pie sobre el carro una vez generada la alteración. Se informó previamente de que se realizaría una desestabilización, sin embargo, los participantes desconocieron el instante de esta y siempre estuvieron aislados del ruido ambiental con audí- fonos. Para la seguridad de los participantes al momento de la alteración utilizaron un sistema de arnés de sujeción al techo del laboratorio. Se realizaron 3 desestabilizaciones por dirección y se aleatorizaron para disminuir el aprendizaje. Se utilizó el promedio de los 3 ensayos de cada dirección para la obten- ción de la latencia muscular. Los datos fueron analizados con el software EMGworks 4.0.9 (Delsys Inc., Boston, MA, EE. UU.). Las señales fueron rectificadas completamente y se aplicó un filtro pasa bajo (cuarto orden, retraso cero, fil- tro Butterworth con una frecuencia de corte de 30 Hz)23. La latencia muscular fue medida en milisegundos (ms) y fue determinada cuando la actividad EMGs sobrepasó un umbral de al menos 3 desviaciones estándar de la media de la señal en reposo de 150 ms de duración23 y se mantuvo este umbral por al menos 25 ms. Análisis estadístico Se utilizó el software estadístico SPSS 14.0 (SPSS 14.0 para Windows, SPSS Inc., IL, EE. UU.) y se calculó la media y desviación estándar para todas las variables. Además, la dis- tribución y la homogeneidad de varianza de los datos fueron determinadas con los test de Shapiro-Wilk y Levene, respec- tivamente. Se utilizó la prueba t de Student para muestras independientes para comparar la latencia muscular entre los grupos. El tamaño del efecto se calculó con la d de Cohen, considerando un efecto pequeño (0,2), moderado (0,5) o fuerte (0,8). Se consideró un nivel alfa de 0,05 para todos los análisis. Resultados En la tabla 1 se observan las características basales de la muestra (edad, peso y talla). Además, uando los participantes fueron expuestos a una desesta- ilización posterior, se observaron diferencias significativas n las latencias de activación de GM (p = 0,0001; d = 2,02) TFL (p < 0,0001; d = 2,22) entre individuos con y sin cortamiento de la BIT. Durante una desestabilización contralateral a la extremi- ad evaluada, se observaron solo diferencias significativas n la latencia del músculo TFL (p < 0,0001; d = 1,85) entre articipantes con y sin acortamiento de la BIT. iscusión ste corresponde al primer estudio en reportar cambios n el control neuromuscular en personas que presentan cortamiento de la BIT sin síndromes musculoesqueléticos sociados. Los resultados de este estudio muestran que los ujetos con acortamiento de la BIT presentan un retraso en a latencia de activación comparados con individuos sin esta ondición, especialmente en dirección antero-posterior. Durante la vida diaria, se generan perturbaciones impre- ecibles que desafían nuestro control motor en diferentes irecciones, incluyendo el plano frontal y sagital. El control euromuscular en ambos planos tiene diferentes patrones e respuesta muscular24. Mientras en el plano sagital la stabilidad es mantenida principalmente por la muscula- ura flexo-extensora, en el plano frontal la musculatura bductora y aductora es la encargada de responder ante lteracionesrepentinas24. En esta investigación, la función rincipal de 3 de los 4 músculos evaluados es en el plano sagi- al (GM, VM y VM). Esto podría explicar por qué los cambios n la latencia de activación en sujetos con acortamiento de a BIT se evidenciaron significativamente en perturbaciones P. Por otra parte, el TFL fue el único músculo que pre- entó diferencias significativas durante desestabilizaciones n el plano frontal. Esto se relaciona con su función prin- ipal como abductor y estabilizador de cadera en el plano rontal25. El cambio de longitud de la BIT afectaría directamente a tensión de los músculos TFL y GM que se encuentran rela- ionados anatómicamente con ella3. McNulty et al. (2004), emostraron que un músculo acortado provoca una dismi- ución de la tasa de descarga de las unidades motoras26. e ha propuesto que la restricción de las longitudes mus- ulares disminuyen los impulsos neurales a los músculos nvolucrados, generando cambios en el control motor (sobre- ctivación, inhibición recíproca, aumento de la latencia uscular)27. Las causas de estos cambios no han sido des- ritas, pero se cree que el cambio de longitud muscular odificaría la información propioceptiva medida por el huso uscular, generando respuestas motoras deficientes frente distintos desafíos motores28. Estas hipótesis ayudan a com- render el retraso en la activación de los músculos TFL y GM bservado en individuos con acortamiento de la BIT. La respuesta tardía de los músculos VL y VM podría ser xplicada por la función dinámica y pasiva que tiene la IT en la articulación patelofemoral. La mayor parte del etináculo lateral de rodilla surge de la BIT, interdigitando lgunas de sus fibras con el VL1. Por lo tanto, la BIT propor- iona indirectamente estabilidad lateral y actúa como un istema de retención pasivo de deslizamiento medial de la 120 E.E. Guzmán-Muñoz et al. Teóricamente, un acortamiento de la BIT podría oca- ionar un deslizamiento, inclinación y compresión lateral de a patela6,11. Cowan et al. (2001) señalan que el mal alinea- iento patelar sería la causa principal del desbalance en la atencia de activación de los músculos VM y VL observado en ujetos con SDFP29. Estos autores indican que el cambio en l control motor no podría ser atribuido a un factor causal, omo el acortamiento de la BIT, debido a que los sujetos valuados presentaban dolor20. En este contexto, nuestros esultados muestran que el acortamiento de la BIT sería un actor que provocaría cambios en el control neuromuscular, ndependientemente de la presencia de dolor. Potencial- ente, el dolor exacerbaría o generaría más cambios en el ontrol motor17. El retraso de la latencia muscular en personas que pre- entan acortamiento de la BIT demuestra la relevancia de antener una longitud muscular normal. Esta alteración odría provocar un déficit en el control neuromuscular de odilla y cadera, aumentando el riesgo de lesiones. Tanto n el SDFP como en el SFBIT, la tensión de BIT ha sido onsiderada como un factor predisponente y un problema ara resolver con la rehabilitación6,10,11. Los resultados de ste estudio indican que, además de incluir ejercicios de exibilidad, deberían ser considerados ejercicios basados n respuestas musculares reflejas con el fin de mejorar los ecanismos de control motor afectados por los cambios de ongitud de la BIT. Dentro de las limitaciones de este estudio se encuentran l pequeño tamaño de la muestra y la selección por conve- iencia de los participantes. Esto probablemente restrinja a validez externa del estudio. A pesar de esto, la signifi- ación estadística y el fuerte tamaño del efecto evidencia l poder de las diferencias observadas en nuestra investi- ación.
