Las complicaciones quirúrgicas son raras pero comprenden dolor o infección en el sitio donde se introduce el generador. Su uso se reserva por lo general para las pacientes que han tenido un tratamiento insatisfactorio con terapia conductual o far- macológica y se ha demostrado que es efectivo para el tratamiento de los síntomas urinarios. En diversos estudios se han notifi cado un índice de mejoría de 60 a 75% y un índice de curación cercano a 45% (Janknegt, 2001; Schmidt, 1999; Siegel, 2000). También se ha demostrado una mejoría sostenida de los parámetros basales de incontinencia en el seguimiento de largo plazo. En un estudio de tres años se observó una reducción de 57% en los episodios de incontinencia por día y en otros estudios de cinco años los hallaz- gos fueron similares (Kerrebroeck, 2007; Siegel, 2000). Asimismo, en la revisión sistemática de 17 series de casos con periodos de seguimiento de tres a cinco años, se identifi có curación en 39% de los pacientes y una mejoría mayor de 50% de los síntomas de incontinencia en 67% (Brazzelli, 2006). Toxina botulínica A La inyección de toxina botulínica A en la pared vesical se puede utilizar como tratamiento de la hiperactividad idiopática del detru- sor. En tres estudios comparativos con placebo se ha demostrado la efi cacia de este tratamiento (Anger, 2010). En los tres se aplicó una inyección cistoscópica de 200 unidades de toxina botulínica A respecto del placebo y en cada una se demostró una mejoría consi- derable de la incontinencia. La mejoría empezó desde cuatro sema- nas después de la inyección (Brubaker, 2008b; Flynn, 2009; Khan, 2010; Sahai, 2007). Un efecto secundario frecuente es la retención urinaria (defi nida como el residuo posmiccional mayor de 200 ml) y se observó en 27 a 43% de las pacientes en estos estudios aleato- rizados. La mayoría de las personas permanece asintomática, pero las que reciben toxina botulínica A por hiperactividad vesical o urgencia urinaria deben comprender que muchas veces necesitarán la colocación de una sonda temporal después de la inyección. Existe la posibilidad de que los efectos de la toxina desaparez- can con el tiempo. En un estudio pequeño en el que se describe la necesidad de varias inyecciones, 20 pacientes de un grupo de 34 recibieron una segunda inyección y nueve pacientes hasta cuatro inyecciones. Estas inyecciones son al parecer tan efectivas como la primera. El intervalo promedio entre las inyecciones es de 377 días (Sahai, 2010).
