Debido a los flujos migratorios, es una enfermedad emer- gente en nuestro medio, donde el reto actual es el cribado en embarazadas y el tratamiento del recién nacido, que es curativo en casi el 100% de los casos, evitando así la enfer- medad crónica y la transmisión a siguientes generaciones1. Se transmite por vía vectorial, transfusional y transpla- centaria. Se presenta el caso de un neonato pretérmino de 32,4 semanas de gestación. Madre boliviana, con serología positiva a Chagas. Ingreso desde la semana 25 por rotura prematura de membranas. Al nacimiento se cursa anatomía patológica de placenta y queda pendiente cribado de Cha- gas mediante PCR al mes de vida. A los 14 días de vida inicia dificultad respiratoria y necesidades de oxigenoterapia, con marcadores de infección y radiografía de tórax normales. ∗ Autor para correspondencia. Ante la clínica sugestiva, el antecedente epidemiológico y la placenta morfológicamente compatible con la enfermedad de Chagas (fig. 1), se inicia tratamiento con benznidazol, obteniéndose confirmación diagnóstica mediante PCR a Try- panosoma cruzi positiva en sangre y líquido cefalorraquídeo. Presenta una buena respuesta clínica y negativización de PCR a los 37 días de vida. El parásito se puede transmitir en cualquier estadio de la infección y cualquier momento del embarazo. Solo un pequeño porcentaje de los recién nacidos infectados tiene sintomatología y la infección aguda pasa inadvertida, entrando en fase crónica2. Aunque la carga parasitaria en la placenta no se rela- ciona con riesgo de transmisión al feto, cuando se acompaña de clínica en el neonato supone un importante apoyo al diagnóstico3. https://doi.org/10.1016/j.anpedi.2019.06.010 1695-4033/© 2020 Asociación Española de Pediatŕıa.
