La extensión fina se realiza para confirm ar un diagnóstico de m alaria, prestando atención en los hem atíes infectados y en los parásitos que hay dentro de ellos. La gota gruesa se observa la m orfología de los parásitos, ya que son m ás abundante que en la ex- tensión y m ás com pactos. Ante un paciente con fiebre paroxística, escalofríos, y sudoración se debe interrogar acerca de: − Antecedentes de exposición, en los últim os 15 días, en áreas con transm isión activa de la enferm edad (ocupación, turism o, desplazam ientos, etc.). − Nexo epidem iológico (tiem po y lugar) con personas que hayan sufrido paludism o. − Antecedentes haber recibido transfusión sanguínea, tejidos u órganos de un paciente con antecedentes de peso epidem iológico. − Antecedentes de m edicación antipalúdica en las últim as cuatro sem anas. − Antecedentes de haber tenido paludism o (recaída de P. vivax hasta 5 años). Los prim eros síntom as de la enferm edad son poco específicos y sim ilares a los de una infección sistém ica de origen viral: dolor de cabeza, debilidad, fatiga, m alestar abdom inal y dolores en articulaciones y m úsculos. El cuadro clínico clásico consiste en accesos febriles precedidos por escalofrío y seguidos de intensa sudoración, y se repiten cada 24, 48 ó 72 horas, según la especie de Plasm odium (Tabla 3). Estos accesos duran entre 6 a 8 horas y ceden abruptam ente: al term inar la sudoración el paciente entra en un período asintom ático, durante el cual sesiente m ejor y aun puede reanudar sus actividades hasta el próxim o acceso febril. En el pasado, dicho com portam iento de la fiebre llevó a denom inar la infección por cada especie según las características de la fiebre: P. vivax: terciana benigna o terciana sim ple, P. m alariae: fiebre cuartana, P. falciparum : terciana m aligna o subterciana. Esa clásica periodicidad de los paroxism os 4 se desarrolla sólo si el paciente no es tratado: si bien, la fiebre interm itente está norm alm ente ausente al com ienzo de la enferm edad puede ser la form a de presentación m ás frecuente. Esposito 1 and for the GeoSentinel Network Abstract Background: More than 30,000 malaria cases are reported annually among international travellers. Despite improve- ments in malaria control, malaria continues to threaten travellers due to inaccurate perception of risk and sub-optimal pre-travel preparation. Methods: Records with a confirmed malaria diagnosis after travel from January 2003 to July 2016 were obtained from GeoSentinel, a global surveillance network of travel and tropical medicine providers that monitors travel-related morbidity. Records were excluded if exposure country was missing or unascertainable or if there was a concomitant acute diagnosis unrelated to malaria. Records were analyzed to describe the demographic and clinical characteristics of international travellers with malaria. Results: There were 5689 travellers included; 325 were children <18 years. More than half (53%) were visiting friends and relatives (VFRs). Most (83%) were exposed in sub-Saharan Africa. The median trip duration was 32 days (interquar- tile range 20–75); 53% did not have a pre-travel visit. More than half (62%) were hospitalized; children were hospital- ized more frequently than adults (73 and 62%, respectively). Ninety-two per cent had a single Plasmodium species diagnosis, most frequently Plasmodium falciparum (4011; 76%). Travellers with P. falciparum were most frequently VFRs (60%). More than 40% of travellers with a trip duration ≤7 days had Plasmodium vivax. There were 444 (8%) travellers with severe malaria; 31 children had severe malaria. Twelve travellers died. Conclusion: Malaria remains a serious threat to international travellers. Efforts must focus on preventive strate- gies aimed on children and VFRs, and chemoprophylaxis access and preventive measure adherence should be emphasized. Background An estimated 214 million infections and 438,000 deaths attributed to malaria occurred globally in 2015 [1]. Although there have been improvements in global malaria control since 2000, malaria remains a threat to inter- national travellers. Malaria is endemic throughout the tropics and sub-tropics, regions visited by an estimated 25–30 million international travellers annually [2], result- ing in an estimated 30,000 travel-related malaria infec- tions [3]. Imported malaria may occur more often along certain travel routes in these areas, and may result in sec- ondary transmission [4] if the infection is brought back to a non-endemic country. La consulta pre-viaje es una herram ienta fundam ental para m inim izar el riesgo de enferm ar. E l 89% inició síntom as en el post viaje y en m ás de la m itad de los casos los síntom as se presentaron antes de los 30 días post- viaje. Factors such as drug efficacy, tolerability [7•], price, and traveler characteristics w ill im pact the choice. T he current priority antim alaria chem oprophylctic regim ens are atovaquone/proguanil, m efloquine, and doxycycline (Fig. 3). C urrentguidelines,such asthe C D C m alaria recom m endations, should be consulted forcorrectuse and dosages ofantim alarial m edication [43]. A ll the priority m edication options are very effective against m alaria, except in som e foci of m ultidrug- resistantm alaria in T haiborder areas.A ntim alarials are,how - ever,associated w ith a high incidence ofadverse events [7•,8] and itis w orthw hile to discuss fears,m yths,and facts regarding these adverse events, since perception w ill im pact traveler behavior and adherence to the chem oprophylaxis. M atching the traveler to the appropriate chem oprophylaxis can be com plex, especially for sm all children [8], travelers w ith com orbidities, and the pregnant traveler. A tovaquone/ proguanil paediatric tablets are a good choice for children (>11 kg in E urope,>5 kg in the U .S.) on shorttrips,or longer term if the budget allow s. M efloquine is a practical, cost- effective option forchildren (>5 kg)[8,9•]w ho stay forlonger periods in m alaria-endem ic areas,butparents need to disguise the bitter taste w ith chocolate or yoghurt.D oxycycline is less frequently used for children (>12 years) (Table 1). O ldertravelers are w idening theirtravelexpectations,often despite com orbidities, and m any questions arise w hen recom m ending chem oprophylaxisw here the travelerisalready taking several m edications for routine conditions. A recent U .S. study of adults 57–85 years of age show ed that 81 % are taking at least one prescription drug, 42 % are taking at leastone O T C drug,and 49 % are using dietary supplem ents. T he results ofthis study w ould suggestthat1 in 25 individuals are atrisk fora m ajordrug–drug interaction.A thorough check of traveler com orbidities and com edication is key. Som e ex- am ples of im portant drug interactions are the follow ing: Travelers on w arfarin m ay need IN R checks or a change of prophylaxis if using doxycycline or atovaquone/proguanil; m efloquine should not be started until after the traveler has com pleted taking his live oraltyphoid fevervaccine.Tables 2, 3,and 4 show possible interactionsthatm ay occurw ith m alaria chem oprophylaxis.
