ACTAS Dermo-Sifiliog resentación del caso a imagen que ilustraba un artículo publicado reciente- ente en la revista Nature1 (reproducida como fig. 1) llamó oderosamente nuestra atención. Aplicando la prueba de orschach con dicha imagen a un virólogo, probablemente xclamaría: ¡coronavirus! Pero nosotros, como dermatólo- os entrenados en dermatoscopia, podríamos responder: ¡un evo melanocítico en crecimiento! O incluso ¡un melanoma! La imagen nos recordó a algunas figuras que hemos visto n determinados nevos melanocíticos displásicos y melano- as incipientes (fig. 2). Cuál es su diagnóstico? orfología marcadamente irregular y polimorfa en grandes structuras pigmentadas periféricas, con frecuencia elon- adas y a menudo de reborde angulado, similares a las spículas de la partícula vírica presentada en la micro- otografía de la figura 1, como marcador de lesiones elanocíticas atípicas en crecimiento (nevos displásicos y elanomas incipientes). omentario a presencia de glóbulos en la periferia de una lesión elanocítica y su significado se describió hace años en la iteratura2. A menudo proyectadas hacia el exterior de su ilueta, incluso a veces sin aparente conexión con ella, uelen ser indicadoras de crecimiento activo3,4. Muchos evos melanocíticos junturales o compuestos en crecimiento uestran un anillo periférico de glóbulos pigmentados urante su proceso de crecimiento. También pueden apa- ecer con frecuencia en nevos de Spitz y de Reed durante su ase de crecimiento más activo5. En ocasiones los glóbulos periféricos pueden estar tam- ién presentes en melanomas. El análisis de la morfología distribución de los glóbulos periféricos ha motivado una ublicación reciente6 en la que no solo la distribución asi- étrica, sino también las variaciones en su forma, tamaño color ---máxime si se dan en lesiones pigmentadas situa- as en las extremidades--- hacen recomendables biopsia o onitorización estrecha, ya que entre las lesiones con estas aracterísticas es posible encontrar algunos melanomas inci- ientes. En su trabajo, Reiter et al.6 no consideraron las lesio- es con pseudópodos que, aunque se proyecten desde la eriferia con una morfología redondeada u ovalada, no son n sentido estricto glóbulos, atendiendo a su definición omo «pequeños cuerpos esféricos» (según la última edición el diccionario de la Real Academia Española de la Len- ua). Asimismo, no creemos que las estructuras pigmentadas eriféricas grandes, elongadas, polimorfas y de reborde casionalmente anguloso que mostramos en los casos que lustran este trabajo (fig. 2) puedan definirse como «glóbulos eriféricos». No pueden llamarse, por tanto, glóbulos sensu tricto y no tienen por qué compartir el significado biológico C ( 73 as 113 (2022) 72---73 i el valor diagnóstico atribuido a los glóbulos pigmentados eriféricos «clásicos» en lesiones melanocíticas2---5. En nuestra experiencia, es poco frecuente esta morfolo- ía tan irregular de las estructuras pigmentadas periféricas, ue no se corresponden claramente con glóbulos ni pseu- ópodos. Hemos identificado recientemente varios casos e nevos melanocíticos con displasia grave y de melano- as incipientes cuyo dato estructural más característico y ospechoso fue la presencia de un anillo periférico, no siem- re completamente cerrado, con este tipo de estructuras fig. 2). A la vista de la microfotografía electrónica (fig. 1), y omo observación realizada en el contexto pandémico en el ue actualmente vivimos, hemos denominado este hallazgo omo: «estructuras pigmentadas periféricas de morfología imilar a las espículas del virus SARS-CoV-2». Las imágenes ue acompañan este trabajo creemos que son suficiente- ente ilustrativas de dicha similitud morfológica. Pensamos ue ni su significado biológico ni su valor diagnóstico se orresponden con el de los glóbulos pigmentados periféri- os «clásicos», aunque esto debería ser evaluado de forma istemática en un número mayor de casos. onflicto de intereses os autores declaran no tener conflictos de interés relacio- ados con el contenido del presente artículo. ibliografía .
