What is the treatment for anaphylaxis supervised by a doctor?
Evaluate quickly (respiratory tract, breathing, circulation, and adequate mental state)
Place in a supine position and elevate lower extremities
Suspend the causal substance or allergen
Inject epinephrine, 1:1000, 0.01 ml/kg (maximum 0.3-0.5 ml) intramuscularly, preferably in the anterolateral region of the thigh or arm (deltoid). Repeat every 5 minutes as necessary to control symptoms and blood pressure. A dilution of epinephrine at 1:10,000 may be used in moribund subjects
Establish and maintain the airway. Racemic epinephrine may be needed by nebulizer in a tracheal tube
Provide supplementary oxygen if necessary (6-8 l/min)
Establish an IV route to maintain blood pressure with IV fluids (saline or plasma expanders), hypertensives (dopamine hydrochloride, 2-10 μ/kg/min, or norepinephrine bitartrate, 2-4 μg/min)
Administer 1.25 mg/kg of diphenhydramine (max. 50 mg) IV over 3-5 minutes
50 mg of ranitidine (1 mg/kg in children) may be diluted in 5% glucose solution up to a total of 20 ml and injected intravenously over 3-5 minutes. Inhaled β-agonists (salbutamol or levalbuterol) or 5 mg/kg of aminophylline in 30 minutes may be useful in severe bronchospasm (0.5-1 mg/kg/h)
Provide 5 mg/kg of hydrocortisone (max. 100 mg) IV every 6 hours or 1-2 mg/kg per 24 hours of methylprednisolone
The treatment for anaphylaxis supervised by a doctor includes evaluating quickly, suspending the causal substance or allergen, and establishing and maintaining the airway.
