275 PROTEINURIA 5. PRONÓSTICO La proteinuria intensa de rango nefrótico se asocia con malos resultados renales en pacientes con enfermedades glomerulares primarias y se- cundarias. Por el contrario, la proteinuria no ne- frótica aislada puede tener un curso mucho más indolente, aunque una fracción de estos pacien- tes eventualmente desarrolla disfunción renal. En pacientes con enfermedad renal crónica, incluso la proteinuria de bajo nivel se asocia con insufi- ciencia renal [23]. Cantidades anormales de proteinuria, incluso albuminuria moderadamente aumentada es un factor de riesgo independiente y significativo para mortalidad por todas las causas, enfermedad car- diovascular y riesgo de enfermedad renal crónica terminal a largo plazo. Su presencia identifica a un grupo de pacientes con un riesgo superior de progresión de la enfermedad renal y con mayor morbilidad cardiovascular; además este riesgo es lineal y continuo, incluso para concentraciones dentro del intervalo de referencia [24].
